Health Permit Requirements
Necessary Information
  • Completed City Application
  • Copy of Health Card (obtained from the doctor / clinic - takes three days to receive card)
  • Copy of Food Handlers Cards
  • Food Management Certification for at least one person (classes are available monthly depending on request - takes three days to obtain certificate)
  • Copy of Sales Tax ID Certificate
  • Grease trap
  • $100 yearly fee

Meat / Meat Products Permits
  • $125 yearly fee
  • Inspections are required twice a year
